Let's add to the list.

The two that come to mind immediately:

Miles Davis - painting (I think he started in later years of his life).

Tony Bennett - painting (not a fan of his pictures, but the man does who he wants)

The great flautist Lloyd McNeill also studied with Pablo Picasso and won several photography awards.  From his obituary:  "McNeill published two volumes of poetry, including “Blackline: A Collection of Poems, Drawings, and Photographs,” and “After the Rain: A Collection of New Poems.” In 2001, he was chosen by the USPS to design a postage stamp for the celebration of Kwanzaa in 2009.".

A.R. Penck (known more as a painter but also recorded prolifically as a drummer, keyboardist, and bassist)

Rodrigo Amado (great photographer in addition to being a heck of tenor player). Vandermark is also an excellent photographer.

Milford Graves (kinetic sculpture)

also going the other way, referencing Penck above, there was the Artist's Jazz Band in Toronto (of which Snow was a member) and the Park Place Gallery jazz band (unrecorded 60s free jazz ensemble comprised of sculptors and painters associated with the gallery). 

yes to Bennink, thanks for the reminder. Also, Misha Mengelberg was loosely associated with Fluxus in its Amsterdam satellite. I don't know how much visual work he did (the ICP 002 maze was his, IIRC) but he was part of that scene in the mid-60s.

Keith Rowe (not sure if you want to call him a jazz musician per se)

Great call! How could I've forgotten! He was a true Renaissance man. I think he was also a President of the American antique microscope collectors society, or something equally esoteric. 

This begs for an addendum: let's see the album covers featuring paintings, drawings, or sculpture (let's throw in the architecture also. After all it is one of the Great Arts) by jazz musicians.

Paul Dunmall

not a jazz musician but Keith Rowe is a long time visual artist with much of his art on numerous AMM album covers as well as his own releases
