Imagine that shoe pictured above, but with the heel at a 45 degree angle, so that the spike heel was coming not out of the heel of the shoe, but out of the instep. It looked like it would be pretty challenging to walk in, but she carried it off flawlessly. I would have given her a standing ovation just for her shoes alone!
